Verity Healthcare Limited is looking to recruit a Care Coordinator. We are a well-established and expanding domiciliary care company providing home carers to clients, helping them to continue living safely and independently in their homes.

Applicants must have held a Care Coordinator position in a domiciliary care company or agency for at least one year and be able to hit the ground running.
The successful candidate will show a high level of commitment and flexibility, able to work autonomously while also collaborating within a team.
Care Coordinator’s Duties
- Accept care packages and assign care workers appropriately.
- Produce care worker rotas and ensure care plans and files are complete.
- Monitor attendance and timeliness using electronic monitoring software.
- Arrange care reviews and risk assessments with the Field Supervisor.
- Conduct telephone monitoring and service user visits to ensure quality.
- Liaise with clients, local authorities and other stakeholders.
- Support the HR Team in recruiting care workers.
- Manage care workers and conduct supervision sessions.
- Represent the company during meetings with officials and commissioners.
- Attend training events to maintain knowledge of industry best practice.
- Ensure compliance with CQC requirements and maintain the highest standards of care.

Experience & Skills (Required)
- At least one year’s experience as a Care Coordinator in a domiciliary care agency.
- Level 2 Diploma for Health and Social Care (QCF).
Desirable Experience & Skills
- Familiarity with Care Planner or similar scheduling software.
- Previous experience as a Care Worker.
