Enablement care services facilitate planned discharge from hospital and detailed support plan for you or your loved one to learn or re-learn skills necessary for daily living.
Reablement care is designed to assist you to develop the confidence to carry out personal and daily activities yourself so that you can continue to live at home independently after an illness or operation. Reablement care focuses on you are as individual with specific outcomes to achieve often with six weeks.
Our specially-trained health care professionals work with you at home, support you or your loved ones to achieve targets agreed when to so you learn new skills or relearn existing ones to help maximise your independence, choice and quality of life.

How we work
Upon receiving a referral, Verity Healthcare assessment team will conduct an assessment and put in place a detailed care plan to ensure the safety of the cared for person while being support to re-learn to do things again. This care offered includes personal care and domestic duties, or it can sometimes just be about providing some company at an unsettling time. We can also provide reablement services to individuals, which is all about helping people do things for themselves so they increase their independence or regain skills following a period of ill health. After 6 weeks, the reablement care many be extended as a continuing care until the person is fully recovered.
We work in partnership with NHS CCG and hospital social workers such as
- Physiotherapists
- Occupational therapists
- Registered nurses
to ensure that you have all the support and resources you need to help you recover as quickly as possible.
Verity Healthcare reablement care helps you to develop confidence for safe transfers including getting up from a chair, in and out of bed and on and off the toilet. Enablement care team will help you acquire safety awareness tips to prevent falls or avoidable accidents reducing risks around the home for you to live independently and safely at home.
What happens after reablement
Reablement care lasts for a relatively short period of time. This can be problematic for the families of those who require continuing care after the six week period. While recovery from operation such as hip replacement is estimated to take 4-6 weeks, traumatic incidents such as terrible falls, strokes and serious injuries can take months to heal to regain normality. In such cases, families may need to consider organising further care and either amongst themselves or by employing a private home carer. . In some cases, the local authority social services may be able to help.
If you or your loved one needs care and support, and reablement care package is not available, or if reablement care package has been given but is not sufficient to aid full recovery, then Verity Healthcare can help.
