We’ll put in place a care plan that’s personally designed in partnership with you or your loved one to ensure that the care plan takes into account your particular needs, goals and aspirations. We’ll set personal outcomes to be achieved. We measure the success of each service in terms of whether those personal outcomes are achieved. This person-centred approach gives our older clients more choice and control over when and how their support is provided. We’ll find a carer or support worker who shares your interests and aspiration so that you’re comfortably supported to derive the best of your care package.
When you’re ready, you’ll receive a welcome care folder containing various information about what to expect from your care. Our branch care manager and the team will visit you to offer you a warm welcome. We’re always ready to amend the care package to suit your changing needs and so you shouldn’t be worried about asking for a review of services if you so wish.

Different options of elderly care
We offer a comprehensive range of elderly care services at home to ensure that we meet the wide range of needs that older people may have. We do not want to leave anyone out. The support and care we provide can range from simple practical everyday living tasks, such as housework, shopping, laundry, correspondence and other paperwork, through to help with personal care such as bathing/showering/ washing, dressing and administering or prompting medication or preparing meals and help with eating and drinking. Our nurse-led team can provide support with even higher-level clinical or nursing care. In many instances, our carers and support workers assist older people to continue to live with their families and enjoy social lives to the full. Our care workers can escort you outside the home to events, visits to hospital, bank or older peoples social centre. They’ll help you to pursue your hobbies and interests. We carefully match carers or support workers who share your interest and aspiration so they can support you appropriately.
It may be that your loved one is recovering from an illness or condition and requires a short term care and support or you’re looking for a long-term solution. Supporting the elderly with nurse-led care
As an organization set up and run by health professionals, we have recruited, trained and maintained a team of registered nursing staff and health professionals such as occupational and physiotherapist who can build bespoke care plans that help people living with dementia, multiple sclerosis, brain injuries, spinal injuries and stroke aftercare. Assisting the elderly with medication

We work with the elderly and the family to ensure that nutritious food and drinks are available to the older person at all times. Our staff understand and ensure that food and drinks are prepare hygienically to avoid contamination, infection and food poising.

Our care workers will also provide advice on healthy eating, assistance with exercise and encourage elderly people to drink water regularly. Sources of funding
There are several sources of funds for practical support and help with day-to-day tasks including state funded support, or private care facilities. The funding sources are intended to help the person with disability to acquire special equipment, adaptations to a home and for home care visits to help with things such as personal care, shopping, cleaning and for recreational purposes.
The first step is to contact the social services at the local council to arrange a health and social care assessment in finding home support for a disabled person. There will be a visit from a social worker and occupational therapist who will state what the disabled person is entitled to and provide a written care assessment and support plan.
There is a direct payments scheme, where the person with disability can access and manage funding for services including employing personal assistant or carer rather than relying on the local council to arrange it. This way, the disabled person has more control over the care services and equipment that the disabled person is able to receive.
