Applicants must have held a Care Coordinator position in a domiciliary care company or agency for at least one year. This role requires a confident individual who can hit the ground running and perform effectively from day one, working both independently and as part of a team.

Key Responsibilities

  • Accept new care packages and assign care workers appropriately to service users.
  • Produce and manage care worker rotas efficiently.
  • Ensure service user care plans and care worker files are fully completed and maintained.
  • Use electronic monitoring systems to track care workers’ attendance and visit timeliness.
  • Arrange care reviews and risk assessments in coordination with field supervisors.
  • Conduct telephone monitoring and service user visits to ensure quality and satisfaction.
  • Liaise with clients, local authorities, and other relevant stakeholders.
  • Support the HR team in recruiting, onboarding, and supervising care workers.
  • Represent the company during meetings with officials and service commissioners.
  • Attend regular training events to maintain and update professional knowledge.
  • Ensure services remain fully compliant with CQC regulations and internal policies.
  • Maintain the highest standards of care at all times.

Experience & Qualifications

  • Minimum one year’s experience as a Care Coordinator in a domiciliary care agency.
  • Level 2 Diploma in Health and Social Care (QCF) or equivalent qualification.
  • Full UK driving licence (mandatory).

Desirable Experience & Skills

  • Familiarity with Care Planner or similar electronic scheduling software.
  • Previous hands-on experience working as a Care Worker.
