Partnership with commissioners

We’re working with many Local Authority Social Services and NHS clinical commissioners to balance capacity with high demand for care.

We’re are working with over 25 NHS Continuing Healthcare (CHC) Commissioning Groups in England as part of the Any  Qualified Provider (AQP) scheme. In addition we work with 18 Local Authority Social Services Commissioning teams to provide a wide range of services to people of all ages and with diverse needs.

Verity Healthcare is an approved and registered healthcare provider for the provision of personal care and treatment of disease, disorder and injuries services (approved by Care Quality Commission) for people of all ages.  As part of the AQP, Verity Healthcare is offering package of homecare services for adults aged 18 or over which is arranged and funded solely by the NHS Clinical Commissioning Groups (CCGs). Our clients who receive CHC funding are initially assessed by Clinical Commissioning Groups (CCGs) according to a legally prescribed decision making process to determine whether the individual has a ‘primary health need’.

Individuals who qualify for AQP, Continuing HealthCare funding are those who:

  • Have a complex medical condition such as dementia and require a lot of care and support
  • Need highly specialised nursing support
  • Are nearing the end of their life and their condition is rapidly getting worse and may be terminal
