Meet Our Nursing Team

Clinical leadership, skilled nursing oversight and compassionate support – helping people receive safe, personalised care at home.   The registered nurses within Verity Healthcare Limited’s Nursing Team bring extensive clinical knowledge, skills and experience, enabling us to provide safe, responsive and personalised care for people with complex and specialist healthcare needs.

Our Clinical Care Team works collaboratively with NHS Continuing Healthcare (CHC) teams, Integrated Care Boards (ICBs), local authorities, hospitals and other healthcare professionals to coordinate responsive and fast-track homecare packages that enable people to receive high-quality clinical support while remaining safely and comfortably in their own homes. The team has extensive experience across health and social care, including NHS services, community healthcare, complex care, long-term condition management and one-to-one nursing support.

CARE PATHWAY

How our nursing team works

A clear journey from referral to review gives families and commissioners confidence in how clinical care is organised.

01

Referral & information

We gather the clinical, social and practical information needed to understand the package.

02

Nursing assessment

A nurse reviews needs, risks, current instructions, equipment and professional involvement.

03

Care & risk planning

The care plan sets out what staff should do, what to monitor and when to escalate.

04

Competency & matching

Staff are selected, trained and competency-assessed for the duties they will perform.

05

Clinical oversight

The nursing team supports implementation, escalation and multidisciplinary communication.

06

Review & improvement

Care is reviewed when needs change and through scheduled quality monitoring.

JOINED-UP CARE

Working with the wider healthcare team

MULTIDISCIPLINARY WORKING

With appropriate consent and information-sharing arrangements, our nurses can liaise with GPs, community nurses, dietitians, pharmacists, speech and language therapists, physiotherapists, occupational therapists, hospital teams, social workers and commissioners.

SAFETY & GOVERNANCE

Clinical quality you can see

Our nursing team helps translate policy, professional guidance and individual clinical instructions into safe day-to-day care.

Competency Assessment

Medication Governance

Nursing oversight supports safe administration, recording, review and incident learning.

Infection Prevention

Clinical practice is supported by hand hygiene, PPE, equipment cleaning and infection-control procedures.

Incident Review

Clinical incidents and near misses can be reviewed to identify immediate actions and wider learning.

Documentation

Clear care records support continuity, accountability, audit and communication across the care team.

Communication

Information is explained in a way the person can understand, including accessible communication where needed.

Relatives and representatives can be involved in line with the person’s wishes and lawful arrangements.
